Even before the final episode of Season 3, I have long thought that Dean would have to have divine help to escape his fate much less hell; guess I was right, grin and wow wasn't Castiel a fantastic affirmation of my theory. What I had initially discounted was that the amulet that Dean wears. Since I had heard in a You-tube interview that it was apparently a random choice by Jensen when being costumed for the pilot, I doubted that it would have any importance to the mythological portion of the story. After I saw the image of the face in the last scene of No Rest for the Wicked my opinion began to change. But at the Chicago con I got to ask Jensen what the prosthetic he wore in that scene looked like. And he confirmed that it did not in any way look like a face. Whether the amulet still has a significant role to play is unknown but right now unless Kripke plays up the illusion we saw into a story line thread (hmmm could the amulet be the reason Anna hears angels) then I think the illusion was accidental. Still in case Kripke ever decides to use it I will leave the meta up as a record. I would like to thank

Below is a screen cap of Dean in hell. Note the right hand lower corner.
I have highlighted the image in yellow. It almost seems to have wings. But on closer examination I realized that the profile actually looked like something I had seen before, Dean's Amulet. Which doesn't mean that it doesn't have wings, its just that wings are of lesser importance than the profile.
Next I took the side view of Dean's amulet from Super-Wicki and overlaid it on the image found on Dean's Shirt. The profile matches almost exactly accept for the open mouth and the horns.
From here I modified the amulet image to open its mouth and cant the head backwards on the neck. Empty spaces were filled in with patterns from the original amulet to smooth out the image
What I find particularly fascinating is how Dean's sweat corresponds to the amulet exhaling. I suspect this represents the "Breath of Life" as many ancient religions as well as Christianity have stories where a divine being breathes life into the dead or the inanimate. If, as it is theorized on Super-Wicki , the amulet is of an Egyptian goddess, BAT, whose name is a feminized version of the Egyptian symbol for Soul then that would seem very appropriate. Another possibility is that the amulet represents Isis. Isis in many images has both HORNS and WINGS and there is a suggestion of wings on Dean's shirt. She would also work well in this role as her spheres of influence include magic, rebirth, the womanly gifts and motherhood causing her to be be in direct opposition to Lilith the Seductress and the destroyer of children. Also unlike Bat who is all but forgotten the mystery cult of Isis lasted well into the 6th century and old images of her are still suspected to be in some European churches masquerading as Black Madonnas. Additionally Isis is known to have assimilated Hathor and her characteristics, who in turn assimilated Bat. Consequently Isis=Hathor=Bat would be a fair conclusion. Equally possible is that the face is the visage of an angel as not every angelic messenger is human. Cherubs have been described as having bull like aspects as well as human faces and Sumarian depictions of their angelic messengers
The only thing I couldn't explain was the horns until I saw this image on the Legacysite.
Note the placement of the meat hook and now compare it with the same image highlighted and a simple amulet overlay. The pointed end of the meat hook forms the horn of the amulet. Even the eye of the hook serves a purpose as it could represent the ring on the amulet.
The image in the last three pictures might not have seemed as compelling as the earlier screen caps, but at a distance the image does stand out as shown on another Legacy site picture below.
Whether Kripke and the writers will take a random choice by the actor and a accidental visual effect and use it to further the story remains to be seen. Regardless the image is creepily accurate for all that it is random.

Tho I've read that the amulet is more mesopotamian, linked to Ishtar. But thematically it still works quite well! She journies to the underworld, shedding clothing as she descends, and retreiving it on the way back. Apparently she was also killed in the underworld, but revived with the waters of life. S4 looks to have something similar - Dean has descended, and the demons will try to take things from him, like hope, humanity. After getting out, he's not quite free, he must retrieve those bits of himself and heal his relationship with Sam. Sam has undoubtedy shed many tears, and perhaps part of that will help Dean be free from the effects of hell. Tho from wikipedia, she could also represent Sam knocking at the gates, demanding his release or else. Plus misfortune and death apparently befell many of her lovers.
Either way, I love the richness of myth in SPN. Simply fantastic!

Poor Lilith; another strong female image literally demonized.
Lilith actually is thought to predate her story with Adam and can be found in Mesopotamian legends as Lilitu a plague and storm demoness. Though it is true Kripke is using the more commonly known version

I never noticed the face profile. Now I will always see it! I did notice that Dean was wearing his amulet in Hell, and I thought that would do something to return him to life.

I guess I need more evidence of Kripke's ability to foreshadow.
What things has he done like this in past episodes? It seems as if he were this clever that we would have seen evidence of it before now.
OTOH, there is that one quick little flash during the opening sequence - during Dean's dream about running from the Hell Hound - where we see him on the floor, dead, straight out of the final scene of this same episode. That, clearly, was stuck in there on purpose - going by TOO quickly to actually be see during the initial airing, someone found it only by doing frame-by-frame. So, yeah, this episode had that, which could suggest there was other stuff in that episode - hidden, obscured stuff - that we have yet to find.
So, is this possibly something Kripke and his evil minions planned and executed - yeah. Will he take credit for it even if it isn't - probably. The evil bastard!

However..I hope this is not how it goes. Because the whole going to hell, coming back thing was already ripped off from Buffy, and the amulet bringing him back would be another rip off. Not exactly the same thing of course but ...if I were Joss Whedon I would be looking over Kripke's shoulder very carefully.
And I say this as a fan of both Supernatural, and Buffy and Angel. It's not that I'm bashing Supernatural, or that I think Buffy or Angel were better shows. Supernatural is the better show. Just Kripke's reuse of an already done storyline (hell, mysterious resurrection, angst), and if he throws in the amulet too....I don't know, that seems to be bordering on plagiarism.
However I don't think he will go that route. Kripke is keeping an eye on the mistakes series like the X-files Buffy Charmed and Angel made and is trying to avoid them. Not that he isn't making his own new mistakes or is being helped into them but he is trying to dodge the old ones
If he does use the amulet I am hoping that it works with out Sam and Bobby's intervention. Maybe just the fact that he has been wearing it all this time will give hims some connection to the spirit it represents - so that when he calls out for help in 3:16 this time something hears and helps him
